Upon submission of the questionnaire, you can now ask your client one or all of the following:
To turn these on, simply go to 'Account' in the main menu > Settings, scroll down and click on 'Questionnaire'.
If you choose to display your terms, the client must agree otherwise they will be unable to submit the questionnaire. However, providing the referral source and consenting to join your mailing list is optional so the client could leave these blank if they choose.
If the client provides a response to where they heard about you and consents to join your mailing list, you can see this information in a number of ways (Note: if these are left blank by the client, nothing will be displayed):
The questionnaire report will display this
Notification bell - shows if a questionnaire has been submitted and will also state if the client has consented to join the mailing list showing their email address
